We thank our partner Nationales Kompetenzzentrum Shared Decision Making / SHARE TO CARE, from whom we have received inspiration, valuable ideas and transfer of online e-learning and other elements adapted for our strategy, taking into consideration local conditions and structures. The contents of these pages are inspired by The Shared Decision Making Summary Guide and the Shared Decision Making (SDM) implementation checklist, both produced by the Shared Decision Making team within the Personalised Care Group at NHS England. We have adapted elements of the framework, as well as the visualization of the process, to make a model for our daily, clinical practice. Also see: National Health Service, Guidance and resources. In addition, we have gained useful insight from the NICE Guideline [NG197] and CFIR framework in the adaptation processes.
